The decision, according to Cyberpunk 2077 loremaster Patrick Mills, was purely practical. The pack was designed for only two characters, one of which had to be Johnny Silverhand. Including both male and female versions of V wasn't feasible. The choice of the female V was a logical consequence, and also a personal preference for Mills.
